For students admitted to Harvard who are currently in the U.S. at another academic institution in F-1 or J-1 non-immigrant status. Part 2 of the form must be completed by an international student advisor in the current institution. The completed form must be submitted to the Admissions Office of the school to which the student has been admitted.

Some international offices refuse to complete SEVIS Transfer-In Forms. If your current international advisor will not complete the transfer-in form, please complete Parts 1 and 2 yourself, and in the “PDSO/DSO/ARO/RO Name” section at the bottom of Part 2, write “my institution will not complete these forms”
