International Student and Scholar Incident Reporting Form

Summary

The Harvard International office (HIO) is required to report to the U.S. Department of State serious situations that could endanger the health, safety, or welfare of an international student or scholar, or otherwise could be expected to bring the Department of State, the Student and Exchange Visitor Program (SEVP), or Harvard's student and exchange visitor program into notoriety or disrepute.  

Harvard administrators should use this form to report serious incidents involving international students or scholars to the Harvard International Office. The HIO will follow up on all incident reports to determine whether further action is needed.
