HIO e-Form Part 1

Summary

This resource is for Harvard and its affiliated hospital administrators to provide information of scholars already in the HIO system (ISD and ISSM) who are changing departments and/or for scholars already in the HIO system from previous stays at Harvard.

Part 1 will be completed by the administrator. Part 2 needs to be completed by the scholar. The administrator will forward both parts to the HIO, along with relevant documentation.
