HIO OPT Request Form

Summary

This form is part of the F-1 OPT application process for Harvard F-1 students. See the F-1 OPT Application Forms Guide for full instructions.

Students must submit the above electronic form to the HIO to request an I-20 with an OPT recommendation. Please note that the earliest date you will receive the I-20 is March 2.

The form will require that students upload the following documents:

1. The HIO Date Verification Form.
2. A copy of your Most Recent I-94 record.
3. For post-completion OPT only, proof of payment for the HIO maintenance fee. 
4. PhD, DrPH, and DDes Students must also upload a completed Doctoral Degree Confirmation Date form.
