GSAS Visa Document Extension/Update Request Form

Summary

F-1 and J-1 students attending the Graduate School of Arts and Sciences should use this form to request an extended I-20 or DS-2019, or a a new I-20 or DS-2019 if you're returning from a leave of absence. 

Please complete the student section of the form and then send it electronically to your department administrator, DGS, or other appropriate official to complete the department section of the form. The fully completed form should be sent to your HIO advisor.

Please note that you must attach supplemental proof of required funding to the form if your departmental funding is insufficient to meet your total expenses.
