Ending STEM OPT Form

Summary

This page provides an overview of the reporting requirements for Harvard F-1 students on STEM OPT. Review the F-1 STEM OPT Reporting Guide for full instructions on reporting requirements and related forms.

Use this form if you are ending your STEM OPT early:

• You have permanently departed the U.S. and you need to end your STEM OPT early.
• You have been admitted to a new program of study in the U.S. and wish to transfer your F-1 status to another institution to begin your new program (or return for a new program at Harvard). You will contact your HIO advisor to coordinate the transfer of your F-1 record so you can begin my new degree program.
• You have changed your immigration status in the U.S., and you need to end your STEM OPT. You will be required to upload documentation of your new immigration status.

A completed Final Evaluation on Student Progress is required (bottom half of page 5 of the I-983).
