#  HIO OPT Request Form 

 



## Summary

This form is part of the [F-1 OPT](/employment-training/opt "F-1 Optional Practical Training (OPT)") application process for Harvard F-1 students. See the [F-1 OPT Application Forms Guide](/forms/opt-application-guide "F-1 OPT Application Forms Guide") for full instructions.

Students must submit the above electronic form to the HIO to request an I-20 with an OPT recommendation. Please note that the earliest date you will receive the I-20 is **March 2**.

The form will require that students upload the following documents:

1. [The HIO Date Verification Form](/forms/opt-date-verification "OPT Date Verification Form").
2. A copy of your Most Recent [I-94 record](https://i94.cbp.dhs.gov/I94/#/home).
3. For post-completion OPT only, [proof of payment for the HIO maintenance fee](https://secure.touchnet.net/C20832_ustores/web/store_main.jsp?STOREID=31&SINGLESTORE=true).
4. **PhD, DrPH, and DDes Students** must also upload a completed [Doctoral Degree Confirmation Date form](/forms/doctoral-degree-confirmation-date "Doctoral Degree Confirmation Date Form").
