Guide to Completing Form I-765
Sections:
| #1 | Your name as it appears on your Form DS-2019. |
| #2 | Leave blank unless your name does not appear on the mailbox. |
| #3 | Your private residential address. If you plan to move in the next 2-4 months please call the HIO. The post office will not forward government mail, so the address used here must be valid for as long as it takes the application to be processed. |
| #4-8 | Self-explanatory |
| #9 | Leave blank if you do not yet have a SSN. You will be eligible for a SSN once your application has been approved. If you already have a SSN, fill it in. |
| #10 | Your 11 digit I-94 card number, found in the top left side of the small white card attached to your passport. |
| #11 | Check “no” if you have never applied for work permission previously. Check “yes” if you have ever applied for work permission. Call the HIO if you have any questions. |
| #12 | This is usually the date stamped in red on the I-94 card (small white card attached to your passport). |
| #13 | As with #12, the location is also stamped in red on the I-94 card. The place is usually written in three letter abbreviations (eg. BOS = Boston). |
| #14 | The status is written in ink on your I94 card (usually J-2). |
| #15 | J-2 |
| #16 | This version of the form has been pre-completed with the J-2 category (C 5) |
