How To Complete Form I-765

Form I-765 here can be filled-in as an Adobe Acrobat form using your computer. You may also print the form and type the information in. Alternatively, you may obtain forms and complete them at your OPT appointment. Most of the questions on this form are easy to answer. If you are unsure how to answer a question, it is better to leave it blank until you meet with your HIO advisor.

Here are some tips for answering particular questions (listed by #) on Form I-765:

  • #1 Your name as it appears on your form I-20. Use UPPER-CASE (capital) letters for your family name.
    (last name/ first name / middle names)
  • #2 Leave blank until your HIO appointment
  • #3 Your private residential address ONLY IF you will be receiving mail there for the next 4-5 months. If not, leave blank until your appointment.
  • #4 As written on your Form I-20
  • #5-#8 (self explanatory)
  • #9 Leave blank if you do not yet have a number. Although a social security number is not required as part of the OPT process, you should apply for a social security number once you have secured employment and no earlier than 30 days before you will begin employment.
  • #10 Your I-94 card number (11 digits) found in the top left side of small white card attached to your passport. If there is a hand-written number on the card, use this number, not the pre-printed one.
  • #11 Leave blank if you are unsure. Check "no" if you have never applied for F-1 OPT before. Check "yes" if you have applied for OPT. The immigration service center serving the northeast is VSC ( Vermont Service Center). List each period of F-1 OPT you have had in the past and fill-in the form using the dates on the EAC(s). You should provide a photocopy of the EAC for each period of OPT granted.
  • #12 Leave blank if unsure. For students who entered the United States with the F-1 visa, the date will be on the I-94 card stapled in the passport (small white card with red ink stamp). Use the date stamped on the I-94 card, even if in some cases (especially with travel to and from Canada) you may have had a more recent entry to the U.S.
  • #13 Also on the I-94 card. As with #12, the location stamped, may not have been the actual place of most recent entry. The place is often written in three letter abbreviations "BOS" ( Boston) "SFR" ( San Francisco). You are permitted to answer the question in general terms, such as "from Canada."
  • #14 If your I-94 card says "F-1", write "student" or "F-1". If not, leave blank until you meet with your HIO advisor
  • #15 Write "student" or "F-1"
  • #16 Fill in (c) (3) (B) for a post-completion OPT application. Leave blank if you are unsure and get the correct code from your HIO advisor later.
