To complete this process you will need your most recently issued Form I-20 or Form DS-2019, and know your SEVIS ID # (found on the top right-hand side of your visa document, above the bar-code). Using the information you submit, the HIO will report changes of address for F and J visa holders to the government database, SEVIS, as required by regulation.
